Appointments are strongly encouraged. Preregister online at https://curative.com/sites/27564
Tests provided at no cost. No prescription or referral needed. Insurance is not required.
However, if you have insurance that will cover the cost of the test, your insurance will be billed. Please bring your insurance card.
