Please complete this form and send it back to the Borough along with the $250.00 sponsorship fee. This fee covers the printing of one full color, double sided, 24” x 50”. banner with pole brackets and installation.
**** Orders will be accepted from July 6th – September 11th. **We need 10 or more applications to print.**
